Hospice Team Services At Family Home Health Network, our interdisciplinary care team is available 24 hours a day, 7 days a week to provide the care and support you and your loved one’s needs. Services Customized care plans Care coordination by a hospice physician Pain and symptom management Social work and counseling services Medical equipment and supplies Emotional and spiritual support for the client and their loved ones Education and community resources Home care services Respite care Bereavement planning and assistance The Hospice Care Team is an interdisciplinary, medically directed team of care providers who collaborate to provide medical, psychological and spiritual support. Team members may include: Hospice Physician Nurses Home Health Aides Social Workers Chaplains or Spiritual Counselors Trained Volunteers
